What To Expect: More Time, More Support, More Understanding

This is physician-guided care, reimagined for the deep work of lifestyle change. Over 6 months, you'll partner with a board-certified physician who acts as both medical expert and coach. We take the time to truly understand you as a whole person - your barriers, your goals, your story- and help you build sustainable change that fits your real life.

During our initial session, we will get to know you and in which areas of health and well-being you want to improve.

We will then create a plan that uses lifestyle interventions to address your specific health concerns.

We will meet every week initially, transitioning to once every other week after 3 months, with access to us through text and email in between as questions or concerns arise.

Each time we meet you will come up with goals that you are ready to take on and are in line with the health outcomes toward which you are working.

We will be there to help guide you and make sure you are on the right track.

As challenges emerge, we will work with you to help determine what can be learned from each experience and how to best move forward.

This is what happens when medical expertise meets coaching support. You get the clinical knowledge to address your specific health conditions, and the intensive guidance to actually implement lasting change. That's why it works: we don't just tell you what to do - we partner with you to figure out how to make it happen in your real life.

There's a lot of confusing information out there when it comes to health advice. Having a qualified physician who knows you and your personal health history provides peace of mind that you are getting trustworthy advice.

Simply telling someone what to do is rarely helpful. Change occurs when it is led by the person doing the changing. While we provide the necessary information, you decide how, when, and why you are making the changes.

The details

The commitment: A 6-month program designed to help you feel better, live better, and take charge of your health- sustainably.

Why 6 months? To make it stick! Six months is considered a minimum amount of time needed to plan, implement, and most importantly begin to maintain behavioral and lifestyle changes in the long run.

Format: A 6-month partnership designed around your needs and your pace.

We begin with an initial 2-hour deep dive session to clarify your goals, explore your story, and build your plan.

From there, we meet weekly for the first 3 months as you build momentum and establish new patterns. In months 4-6, we transition to biweekly sessions as you gain confidence and autonomy.

Between sessions, you have direct email and text access for support, questions, and accountability.

You will work with either Laura Grangeia, MD or Justin ("Jay") Stone, MD.

Problem solved: When you’re constantly managing symptoms and juggling medications, it can feel like your health is something happening to you- not something you have influence over.

Through this program, we shift the narrative. You’ll learn how to use food, movement, rest, connection, and meaning as tools for healing. Together, we’ll explore ways to reduce medication dependence (when appropriate), support possible reversal of chronic diagnoses, and rebuild trust in your body and mind and their ability to thrive.

  • Visit 1: Wellness Vision and Goal Setting

  • Visits 2-11: Personalized Goal Review and Action Planning

  • Visit 12: 3-month Check-in and Recalibration

  • Visits 13-17: Personalized Goal Review and Action Planning

  • Visit 18: 6-month Progress Review and Plan for Future

Transformation/Results: The transformation will look different for everyone- because it’s built around you. This is a personalized process, shaped by your body, your goals, and your level of commitment.

For some, it means reducing or even coming off medications. For others, it might include reversing- or significantly improving- a chronic condition. And for many, it’s about preventing illness before it starts.

As we're building our lifestyle medicine practice, we're offering a reduced introductory rate for our founding clients. This rate will increase as our practice grows.

Consider the 3-Month Foundation Track

Not everyone needs or is ready for a 6-month deep dive- and that’s okay. The Foundations Track is designed for those who want to get started, build momentum, and begin seeing real shifts in their health and energy.

This is a physician-led program that includes personalized medical advice, disease-specific guidance, and integrative coaching to support your long-term health—whether you're managing a diagnosis or aiming to prevent one.


What’s Included:

  • 1 initial 120-minute session to clarify goals and create your personalized action plan

  • 11 weekly coaching sessions (60 minutes each)

  • Email/text access between sessions for questions and support

  • Tailored guidance in nutrition, movement, breathwork, sleep, stress regulation, and behavior change

  • Clear next steps at the end of the program, including options to continue or maintain your progress independently
